Sports injury: when should you see a physiotherapist?

Understand when a sports injury needs medical attention, when physiotherapy may help, and what to discuss before returning to training.

Published by Harrison Zhang, MScPT

At a glance

If an injury keeps interrupting normal movement or your return to training, a physiotherapy assessment can help you discuss the next step. Some injuries need urgent medical assessment first. The decision should depend on the symptoms and the activity affected, rather than waiting a fixed number of days.

When to seek medical attention first

After an injury, seek urgent medical assessment for severe or worsening pain, rapidly increasing swelling, or inability to bear weight. Go to an emergency department for obvious deformity, new numbness, or a cold or discoloured limb. Call 911 for an emergency or if you cannot get there safely. Do not wait for a routine physio appointment.

When a physiotherapy assessment may be useful

A sprain or strain that is not settling may benefit from physiotherapy. An assessment is an opportunity to clarify what is limiting you and whether another healthcare professional should be involved. It is not possible to identify every injury from a description online.

When you contact the clinic, describe a specific difficulty: stairs, walking to work, lifting a weight, or changing direction on court. Also say whether the problem is improving, staying the same or returning each time you train. These details make the conversation more useful than a pain score alone.

Returning to sport involves more than a date

For an ankle sprain, for example, rehabilitation commonly progresses through movement, strength and balance work before more demanding sport activities. The AAOS describes a gradual return, with turning and cutting activities introduced later. This is an example of a staged approach, not a programme for every injury.

Ask what would show that you are ready for your next training step. For a basketball player, the discussion might include returning to practice and then a game; for a runner, it might focus on the next manageable running session. The actual progression needs to fit your assessment and sport.

Four questions to bring to your appointment

Use the appointment to get a plan you can follow between visits. It helps to bring your training schedule, any competition dates, and a description of what has changed since the injury.

  • Which activities can I continue, and which should I change for now?
  • What response to an exercise should make me stop or contact you?
  • What should we check before increasing my training?
  • When will we review the plan if progress is slower than expected?
