Back pain & mobility

When should you see a physiotherapist for low back pain?

Know when low back pain may benefit from physiotherapy, which symptoms need medical attention first, and what to expect at a Markham assessment.

At a glance

Consider booking a physiotherapy assessment when low back pain is limiting sitting, walking, lifting, sleep, work or exercise; keeps returning; or is not improving as expected. You do not need to wait until the pain becomes severe. New bladder or bowel changes, numbness around the groin, rapidly worsening weakness, significant trauma, or feeling unwell with fever need medical assessment first.

Which symptoms need medical attention first?

Go to an emergency department for back pain with new difficulty urinating, loss of bladder or bowel control, numbness around the groin or anus, or rapidly worsening weakness in one or both legs. Call 911 for an emergency or if you cannot get there safely. Do not wait for a routine physiotherapy appointment.

Arrange prompt medical assessment after significant trauma, or when back pain occurs with fever, marked illness, a history of cancer, unexplained weight loss, osteoporosis, or long-term steroid use. These details do not confirm a serious condition, but they can change what should be checked and how quickly.

Five practical reasons to book a physiotherapy assessment

The decision is less about reaching a fixed pain score or waiting a set number of days, and more about how the problem is affecting your function and whether you have a clear plan.

  • Pain is changing how you sit, walk, sleep, work, lift, exercise or care for family.
  • The same episode keeps returning when you resume normal activity or training.
  • You are improving, but do not know how to safely rebuild strength or return to the gym, running or sport.
  • You have stable pain, tingling or numbness into the buttock or leg and want it assessed. New or worsening weakness needs more urgent medical attention.
  • The problem is not settling, or online advice has left you unsure which movements to continue.

What happens during a low back pain assessment?

The assessment starts with your story: when symptoms began, what changes them, your health history, and the activities you want to regain. The physical examination may include relevant spinal and hip movement, strength, balance, functional tasks and a neurological screen when leg symptoms are present. Not everyone needs the same tests.

The purpose is to identify what is limiting you, screen for findings that may need another healthcare professional, and agree on a manageable next step. Physiotherapy should involve you in decisions about your own recovery.

Will treatment be exercise, hands-on care, or both?

There is no single treatment that fits every episode of back pain. A plan may include education, activity modification and progressive exercise. When appropriate, manual therapy such as mobilisation or soft-tissue techniques can be considered as part of a package that also includes active rehabilitation—not as a stand-alone promise to fix the problem.

At Harrison Zhang Physio, hands-on care may be used to help create a more comfortable window for movement. The longer-term goal is to help you understand the problem, rebuild capacity and return to the activities that matter to you.

Do you need an X-ray or MRI before physiotherapy?

Most uncomplicated new low back pain does not require routine imaging before an assessment. Imaging is more useful when the history and examination suggest a specific concern and the result would change care. Scans can also show common age-related changes that may not explain the symptoms.

If you already have a report, bring it to your appointment. If your symptoms or examination suggest further medical investigation, Harrison can discuss why follow-up may be appropriate. Read the website’s X-ray guide for a fuller explanation.

How many physiotherapy visits will you need?

There is no responsible way to promise a visit count before understanding the problem. The number and spacing of appointments depend on your findings, goals, response to treatment and ability to work on the plan between visits. A useful first appointment should still leave you with a clearer explanation, a practical next step and a way to review progress.
